VIA Care

VIA Care provide Support Coordination, Nursing supports and the full range of Core Supports (SIL, Assistance with Daily Living, Social and community participation etc). The practice is heavily informed by our leadership’s background in social work, meaning that we operate in a developmentally and trauma informed manner which is responsive to people’s family, social and economic circumstances. We are accustomed to working with families also engaged with child protection and family violence and have managed a multitude of hospital to home transitions following lengthy hospital stays.
